Senior Monthly General Medical Care is a program created by L.I.N.K.S Community and Family Services that will provide seniors with general medical care. The common goal of this program is to aid seniors in maintaining an independent lifestyle for as long and safely possible.
- Seniors will first have their physical ability evaluated.
- Staff will suggest personalized exercise routines based upon these evaluations.
- Twelve monthly sessions will be provided.

Medication Review
Monitoring of weight, blood pressure, temperature, and blood sugar
General Health Assessment
Nutritional Recommendations
- Home safety assessment and minor modifications will be suggested such as: grab bars, commode rails, etc.
- There is no charge to the senior for this service.
- L.I.N.K.S hosts monthly group activities
WHO CAN PARTICIPATE?
- All seniors that have residency in or around the Akron area
- Seniors 58 years and older
ELIGIBILITY
- Have been dependent on the income of another family member but are no longer supported by that income.
- Dependent on federal assistance
- Supported by public assistance or spousal support.
- Income is below $30,000 a year.
WHAT ARE THE THREE COMPONENTS OF THE PROGRAM?
- Prevention ideals and educational seminars
- General health checks by license RN or license Pharmacist
- Support team that will check-up on senior throughout the month

- Contact us by phone. L.I.N.K.S Community and Family Services, Charles Martin (Program Director). We will need the following information: Name, D.O.B, and phone number. All client information is confidential and handled according to HIPPA guidelines.
